(Mometasone Furoate / Olopatadine Hydrochloride)
Ryaltris is a prescription nasal spray that combines two active ingredients:
Olopatadine hydrochloride, a selective histamine H1-receptor antagonist (antihistamine), and
Mometasone furoate, a corticosteroid that reduces inflammation.
Ryaltris is indicated for the treatment of symptoms of seasonal allergic rhinitis (SAR) in patients 12 years of age and older, including nasal congestion, sneezing, nasal itching, and runny nose. It works by blocking histamine-mediated allergic responses and reducing the production of inflammatory cytokines in the nasal mucosa.
Note: Ryaltris is for intranasal use only. Do not spray into the eyes or mouth.
Ryaltris is prescribed to relieve seasonal allergic rhinitis symptoms, such as:
Nasal congestion
Sneezing
Runny nose
Nasal itching
Post-nasal drip
It is particularly useful for patients who require both an antihistamine and a steroid to help manage allergy symptoms.
FDA-approved use: Treatment of SAR in adults and children 12 years of age and older. Not approved for use in children under 12.

Shake the bottle gently before each use.
Dosage: 2 sprays in each nostril twice daily (morning and evening) is often recommended. Do not administer the spray anywhere other than in the nostrils.
Prime (spray away from your face and ensure a mist is visible) the pump before first use or if it hasn’t been used for 14 days or more.
Gently blow your nose before spraying.
Tilt your head slightly forward, insert the spray tip into one nostril, and aim slightly outward.
Avoid sneezing or blowing your nose immediately after use.
Do not exceed the prescribed dose. If swallowed or if overdose is suspected, seek emergency medical attention immediately.
Before starting Ryaltris, tell your healthcare provider if you have or have had:
Recent nasal surgery, nasal trauma, or open sores in the nose (increased risk of delayed wound healing or septal perforation)
Glaucoma or cataracts (due to corticosteroid exposure)
Tuberculosis, fungal, bacterial, or viral infections, especially untreated or systemic
Ocular herpes simplex (risk of viral reactivation)
Immunosuppressive conditions or exposure to chickenpox or measles
Adrenal insufficiency or HPA (hypothalamic-pituitary-adrenal) axis suppression
Pregnancy: Safety has not been established. Consult your healthcare provider to determine whether the benefits of use are likely to outweigh potential risks.
Breastfeeding: The safety of Ryaltris for use during breastfeeding is unknown.
Ryaltris may interact with CYP3A4 inhibitors, which can increase systemic exposure to corticosteroids. A variety of prescription and over-the-counter products may interact with CYP3A4 inhibitors, so it’s important to talk with your healthcare provider about all of the products you use to help ensure they can be used safely together.
There are no specific food or drink restrictions while using Ryaltris. However, it is generally recommended to:
Avoid alcohol and sedatives, as they may increase drowsiness.
Maintain general hydration, particularly during allergy season.
Use the missed dose as soon as you remember.
If it's close to the time of your next dose, skip the missed dose.
Do not double the dose to catch up.
Consistency improves effectiveness, especially during allergy season.
Serious side effects (less common, but it’s critical to seek medical care immediately):
Nasal septal perforation (hole in nasal cartilage): symptoms include crusting, whistling sound, nosebleeds
Delayed wound healing after nasal injury or surgery
Fungal infection (thrush) in the nose/throat: white patches, redness, discomfort
Allergic reactions: rash, hives, wheezing, swelling of face/tongue/throat, difficulty breathing
Glaucoma or cataracts: visual changes, eye pain—requires regular eye exams
Adrenal insufficiency: fatigue, weakness, nausea, low blood pressure
Immunosuppression: increased risk of infection—avoid exposure to chickenpox/measles
Severe drowsiness or CNS effects
Common side effects:
Unpleasant taste
Nasal discomfort or irritation
Nosebleeds (epistaxis)
These are not all possible side effects. Report adverse events to the FDA at 1-800-FDA-1088 or visit www.fda.gov/medwatch.
Store at room temperature, between 68°F and 77°F (20°C and 25°C).
Protect from light and moisture.
Keep the bottle upright and tightly closed when not in use.
Discard the bottle after 240 sprays (as indicated on the label), even if medication remains.
Ryaltris may interact with:
CYP3A4 inhibitors, including ritonavir, cobicistat, ketoconazole, and clarithromycin
CYP3A4 substrates, including antifungals, antibiotics, and some antidepressants
Avoid driving or operating heavy machinery until you know how Ryaltris affects you. Ryaltris may cause drowsiness.
Let your healthcare provider know if your symptoms do not improve after a few days of regular use.
Not intended for acute relief of severe nasal congestion or allergic reactions.
Keep a current list of medications and show it to your healthcare provider and pharmacist during each visit.
